b3ae209697 KVM: Introduce kvm_unmap_hva_range() for kvm_mmu_notifier_invalidate_range_start()
When we tested KVM under memory pressure, with THP enabled on the host,
we noticed that MMU notifier took a long time to invalidate huge pages.

Since the invalidation was done with mmu_lock held, it not only wasted
the CPU but also made the host harder to respond.

This patch mitigates this by using kvm_handle_hva_range().

e1612de9e4 powerpc: Disable /dev/port interface on systems without an ISA bridge
Some power systems do not have legacy ISA devices. So, /dev/port is not
a valid interface on these systems. User level tools such as kbdrate is
trying to access the device using this interface which is causing the
system crash.

This patch will fix this issue by not creating this interface on these
powerpc systems.

18ad51dd34 powerpc: Add VDSO version of getcpu
We have a request for a fast method of getting CPU and NUMA node IDs
from userspace. This patch implements a getcpu VDSO function,
similar to x86.

Ben suggested we use SPRG3 which is userspace readable. SPRG3 can be
modified by a KVM guest, so we save the SPRG3 value in the paca and
restore it when transitioning from the guest to the host.

I have a glibc patch that implements sched_getcpu on top of this.
Testing on a POWER7:

baseline: 538 cycles
vdso:      30 cycles

d6b9a81b2a powerpc: IOMMU fault injection
Add the ability to inject IOMMU faults. We enable this per device
via a fail_iommu sysfs property, similar to fault injection on other
subsystems.

An example:

The logic to choose whether to use the SIAR or get the information
out of pt_regs is going to get more complicated, so do it once in
perf_read_regs.

We overload regs->result which is gross but we are already doing it

cdaade7129 powerpc: Start using ___PPC_RA/B/S/T where necessary
Now have ___PPC_RA/B/S/T we can use it in some places.  These are
places where we can't use the existing defines which will soon enforce
R0-R31 usage.

The macros being changed here are being used in inline asm, which
can't convert to enforce the R0-R31 usage.

bpf_jit uses a mix of both generated and non-generated with the same
code, so just convert all these to use the ___PPC_R versions which
won't enforce R usage later.

There was a typo, checking for CONFIG_TRACE_IRQFLAG instead of
CONFIG_TRACE_IRQFLAGS causing some useful debug code to not be
built

This in turns causes a build error on BookE 64-bit due to incorrect
semicolons at the end of a couple of macros, so let's fix that too

be2cf20a5a powerpc: More fixes for lazy IRQ vs. idle
Looks like we still have issues with pSeries and Cell idle code
vs. the lazy irq state. In fact, the reset fixes that went upstream
are exposing the problem more by causing BUG_ON() to trigger (which
this patch turns into a WARN_ON instead).

We need to be careful when using a variant of low power state that
has the side effect of turning interrupts back on, to properly set
all the SW & lazy state to look as if everything is enabled before
we enter the low power state with MSR:EE off as we will return with
MSR:EE on. If not, we have a discrepancy of state which can cause
things to go very wrong later on.

This patch moves the logic into a helper and uses it from the
pseries and cell idle code. The power4/970 idle code already got
things right (in assembly even !) so I'm not touching it. The power7
"bare metal" idle code is subtly different and correct. Remains PA6T
and some hypervisor based Cell platforms which have questionable
code in there, but they are mostly dead platforms so I'll fix them
when I manage to get final answers from the respective maintainers
about how the low power state actually works on them.

b4c3a8729a powerpc/iommu: Implement IOMMU pools to improve multiqueue adapter performance
At the moment all queues in a multiqueue adapter will serialise
against the IOMMU table lock. This is proving to be a big issue,
especially with 10Gbit ethernet.

This patch creates 4 pools and tries to spread the load across
them. If the table is under 1GB in size we revert back to the
original behaviour of 1 pool and 1 largealloc pool.

We create a hash to map CPUs to pools. Since we prefer interrupts to
be affinitised to primary CPUs, without some form of hashing we are
very likely to end up using the same pool. As an example, POWER7
has 4 way SMT and with 4 pools all primary threads will map to the
same pool.

The largealloc pool is reduced from 1/2 to 1/4 of the space to
partially offset the overhead of breaking the table up into pools.

Some performance numbers were obtained with a Chelsio T3 adapter on
two POWER7 boxes, running a 100 session TCP round robin test.

Performance improved 69% with this patch applied.

all do the same thing so use DECLARE_EVENT_CLASS to avoid duplicating

The following patch is to remove the pseries_notify_add_cpu() call
and replace it by a hot plug notifier.

This would prevent cpuidle resources being released and allocated each
time cpu comes online on pseries.

The earlier design was causing a lockdep problem

b6e3796834 powerpc: Have patch_instruction detect faults
For ftrace to use the patch_instruction code, it needs to check for
faults on write. Ftrace updates code all over the kernel, and we need to
know if code is updated or not due to protections that are placed on
some portions of the kernel. If ftrace does not detect a fault, it will
error later on, and it will be much more difficult to find the problem.

By changing patch_instruction() to detect faults, then ftrace will be
able to make use of it too.

0b17ba7258 powerpc: check_and_cede_processor() never cedes
Commit f948501b36 ("Make hard_irq_disable() actually hard-disable
interrupts") caused check_and_cede_processor to stop working.
->irq_happened will never be zero right after a hard_irq_disable
so the compiler removes the call to cede_processor completely.

The bug was introduced back in the lazy interrupt handling rework
of 3.4 but was hidden until recently because hard_irq_disable did
nothing.

This issue will eventually appear in 3.4 stable since the
hard_irq_disable fix is marked stable, so mark this one for stable
too.

f948501b36 Make hard_irq_disable() actually hard-disable interrupts
At present, hard_irq_disable() does nothing on powerpc because of
this code in include/linux/interrupt.h:

    #ifndef hard_irq_disable
    #define hard_irq_disable()      do { } while(0)
    #endif

So we need to make our hard_irq_disable be a macro.  It was previously
a macro until commit 7230c56441 ("powerpc: Rework lazy-interrupt
handling") changed it to a static inline function.

32fad281c0 KVM: PPC: Book3S HV: Make the guest hash table size configurable
This adds a new ioctl to enable userspace to control the size of the guest
hashed page table (HPT) and to clear it out when resetting the guest.
The KVM_PPC_ALLOCATE_HTAB ioctl is a VM ioctl and takes as its parameter
a pointer to a u32 containing the desired order of the HPT (log base 2
of the size in bytes), which is updated on successful return to the
actual order of the HPT which was allocated.

There must be no vcpus running at the time of this ioctl.  To enforce
this, we now keep a count of the number of vcpus running in
kvm->arch.vcpus_running.

If the ioctl is called when a HPT has already been allocated, we don't
reallocate the HPT but just clear it out.  We first clear the
kvm->arch.rma_setup_done flag, which has two effects: (a) since we hold
the kvm->lock mutex, it will prevent any vcpus from starting to run until
we're done, and (b) it means that the first vcpu to run after we're done
will re-establish the VRMA if necessary.

If userspace doesn't call this ioctl before running the first vcpu, the
kernel will allocate a default-sized HPT at that point.  We do it then
rather than when creating the VM, as the code did previously, so that
userspace has a chance to do the ioctl if it wants.

When allocating the HPT, we can allocate either from the kernel page
allocator, or from the preallocated pool.  If userspace is asking for
a different size from the preallocated HPTs, we first try to allocate
using the kernel page allocator.  Then we try to allocate from the
preallocated pool, and then if that fails, we try allocating decreasing
sizes from the kernel page allocator, down to the minimum size allowed
(256kB).  Note that the kernel page allocator limits allocations to
1 << CONFIG_FORCE_MAX_ZONEORDER pages, which by default corresponds to
16MB (on 64-bit powerpc, at least).

55ccf3fe3f fork: move the real prepare_to_copy() users to arch_dup_task_struct()
Historical prepare_to_copy() is mostly a no-op, duplicated for majority of
the architectures and the rest following the x86 model of flushing the extended
register state like fpu there.

Remove it and use the arch_dup_task_struct() instead.

ffe3649282 powerpc/kvm: Fix VSID usage in 64-bit "PR" KVM
The code forgot to scramble the VSIDs the way we normally do
and was basically using the "proto VSID" directly with the MMU.

This means that in practice, KVM used random VSIDs that could
collide with segments used by other user space programs.

f2ab621996 powerpc/pseries: Add PFO support to the VIO bus
Add support for the Platform Facilities Option (PFO) to the VIO bus.
These devices have a separate root node in OpenFirmware which
requires additional parsing to map into the existing VIO device
structure fields. This adds the interface for PFO device drivers to
make synchronous hypervisor calls.

7563bbf89d gpiolib/arches: Centralise bolierplate asm/gpio.h
Rather than requiring architectures that use gpiolib but don't have any
need to define anything custom to copy an asm/gpio.h provide a Kconfig
symbol which architectures must select in order to include gpio.h and
for other architectures just provide the trivial implementation directly.

This makes it much easier to do gpiolib updates and is also a step towards
making gpiolib APIs available on every architecture.

For architectures with existing boilerplate code leave a stub header in
place which warns on direct inclusion of asm/gpio.h and includes
linux/gpio.h to catch code that's doing this.  Direct inclusion of
asm/gpio.h has long been deprecated.

cb83b629ba sched/numa: Rewrite the CONFIG_NUMA sched domain support
The current code groups up to 16 nodes in a level and then puts an
ALLNODES domain spanning the entire tree on top of that. This doesn't
reflect the numa topology and esp for the smaller not-fully-connected
machines out there today this might make a difference.

Therefore, build a proper numa topology based on node_distance().

Since there's no fixed numa layers anymore, the static SD_NODE_INIT
and SD_ALLNODES_INIT aren't usable anymore, the new code tries to
construct something similar and scales some values either on the
number of cpus in the domain and/or the node_distance() ratio.

54771e6217 KVM: PPC: Emulator: clean up SPR reads and writes
When reading and writing SPRs, every SPR emulation piece had to read
or write the respective GPR the value was read from or stored in itself.

This approach is pretty prone to failure. What if we accidentally
implement mfspr emulation where we just do "break" and nothing else?
Suddenly we would get a random value in the return register - which is
always a bad idea.

So let's consolidate the generic code paths and only give the core
specific SPR handling code readily made variables to read/write from/to.

Functionally, this patch doesn't change anything, but it increases the
readability of the code and makes is less prone to bugs.

f31e65e117 kvm/book3s: Make kernel emulated H_PUT_TCE available for "PR" KVM
There is nothing in the code for emulating TCE tables in the kernel
that prevents it from working on "PR" KVM... other than ifdef's and
location of the code.

This and moves the bulk of the code there to a new file called
book3s_64_vio.c.

This speeds things up a bit on my G5.

6e35994d1f KVM: PPC: Use clockevent multiplier and shifter for decrementer
Time for which the hrtimer is started for decrementer emulation is calculated
using tb_ticks_per_usec. While hrtimer uses the clockevent for DEC
reprogramming (if needed) and which calculate timebase ticks using the
multiplier and shifter mechanism implemented within clockevent layer.

It was observed that this conversion (timebase->time->timebase) are not
correct because the mechanism are not consistent.
In our setup it adds 2% jitter.

With this patch clockevent multiplier and shifter mechanism are used when
starting hrtimer for decrementer emulation. Now the jitter is < 0.5%.

ec34a68149 powerpc: Remove old powerpc specific ptrace getregs/setregs calls
PowerPC has non standard getregs calls that only dump the GPRs or
FPRs and have their arguments reversed. commit e17666ba48 (ptrace
updates & new, better requests) in 2.6.3 deprecated them and introduced
more standard versions.

It's been about 5 years and I know of no users of the old calls so
lets remove them.

Remove CONFIG_POWER4_ONLY, the option is badly named and only does two
things:

- It wraps the MMU segment table code. With feature fixups there is
  little downside to compiling this in.

- It uses the newer mtocrf instruction in various assembly functions.
  Instead of making this a compile option just do it at runtime via
  a feature fixup.

35000870fc powerpc: Optimise enable_kernel_altivec
Add two optimisations to enable_kernel_altivec:

- enable_kernel_altivec has already determined if we need to
save the previous task's state but we call giveup_altivec
in both cases, requiring an extra branch in giveup_altivec. Create
giveup_altivec_notask which only turns on the VMX bit in the
MSR.

- We write the VMX MSR bit each time we call enable_kernel_altivec
even it was already set. Check the bit and branch out if we have
already set it. The classic case for this is vectored IO
where we have to copy multiple buffers to or from userspace.

The following testcase was used to confirm this patch improves
performance:

Since the current breakpoint for using VMX in copy_tofrom_user is
4096 bytes, I'm using buffers of 4096 + 1 cacheline (4224) bytes.
A benchmark of 16 entry readvs (-s 16):

time copy_to_user -l 4224 -s 16 -i 1000000

completes 5.2% faster on a POWER7 PS700.

4013369f37 powerpc/irqdomain: Fix broken NR_IRQ references
The switch from using irq_map to irq_alloc_desc*() for managing irq
number allocations introduced new bugs in some of the powerpc
interrupt code.  Several functions rely on the value of NR_IRQS to
determine the maximum irq number that could get allocated.  However,
with sparse_irq and using irq_alloc_desc*() the maximum possible irq
number is now specified with 'nr_irqs' which may be a number larger
than NR_IRQS.  This has caused breakage on powermac when
CONFIG_NR_IRQS is set to 32.

This patch removes most of the direct references to NR_IRQS in the
powerpc code and replaces them with either a nr_irqs reference or by
using the common for_each_irq_desc() macro.  The powerpc-specific
for_each_irq() macro is removed at the same time.

Also, the Cell axon_msi driver is refactored to remove the global
build assumption on the size of NR_IRQS and instead add a limit to the
maximum irq number when calling irq_domain_add_nomap().

6020c0f6e7 KVM: PPC: Pass EA to updating emulation ops
When emulating updating load/store instructions (lwzu, stwu, ...) we need to
write the effective address of the load/store into a register.

Currently, we write the physical address in there, which is very wrong. So
instead let's save off where the virtual fault was on MMIO and use that
information as value to put into the register.

While at it, also move the XOP variants of the above instructions to the new
scheme of using the already known vaddr instead of calculating it themselves.

0456ec4ff2 KVM: PPC: Book3S HV: Report stolen time to guest through dispatch trace log
This adds code to measure "stolen" time per virtual core in units of
timebase ticks, and to report the stolen time to the guest using the
dispatch trace log (DTL).  The guest can register an area of memory
for the DTL for a given vcpu.  The DTL is a ring buffer where KVM
fills in one entry every time it enters the guest for that vcpu.

Stolen time is measured as time when the virtual core is not running,
either because the vcore is not runnable (e.g. some of its vcpus are
executing elsewhere in the kernel or in userspace), or when the vcpu
thread that is running the vcore is preempted.  This includes time
when all the vcpus are idle (i.e. have executed the H_CEDE hypercall),
which is OK because the guest accounts stolen time while idle as idle
time.

Each vcpu keeps a record of how much stolen time has been reported to
the guest for that vcpu so far.  When we are about to enter the guest,
we create a new DTL entry (if the guest vcpu has a DTL) and report the
difference between total stolen time for the vcore and stolen time
reported so far for the vcpu as the "enqueue to dispatch" time in the
DTL entry.

2e25aa5f64 KVM: PPC: Book3S HV: Make virtual processor area registration more robust
The PAPR API allows three sorts of per-virtual-processor areas to be
registered (VPA, SLB shadow buffer, and dispatch trace log), and
furthermore, these can be registered and unregistered for another
virtual CPU.  Currently we just update the vcpu fields pointing to
these areas at the time of registration or unregistration.  If this
is done on another vcpu, there is the possibility that the target vcpu
is using those fields at the time and could end up using a bogus
pointer and corrupting memory.

This fixes the race by making the target cpu itself do the update, so
we can be sure that the update happens at a time when the fields
aren't being used.  Each area now has a struct kvmppc_vpa which is
used to manage these updates.  There is also a spinlock which protects
access to all of the kvmppc_vpa structs, other than to the pinned_addr
fields.  (We could have just taken the spinlock when using the vpa,
slb_shadow or dtl fields, but that would mean taking the spinlock on
every guest entry and exit.)

This also changes 'struct dtl' (which was undefined) to 'struct dtl_entry',
which is what the rest of the kernel uses.

f0888f7015 KVM: PPC: Book3S HV: Make secondary threads more robust against stray IPIs
Currently on POWER7, if we are running the guest on a core and we don't
need all the hardware threads, we do nothing to ensure that the unused
threads aren't executing in the kernel (other than checking that they
are offline).  We just assume they're napping and we don't do anything
to stop them trying to enter the kernel while the guest is running.
This means that a stray IPI can wake up the hardware thread and it will
then try to enter the kernel, but since the core is in guest context,
it will execute code from the guest in hypervisor mode once it turns the
MMU on, which tends to lead to crashes or hangs in the host.

This fixes the problem by adding two new one-byte flags in the
kvmppc_host_state structure in the PACA which are used to interlock
between the primary thread and the unused secondary threads when entering
the guest.  With these flags, the primary thread can ensure that the
unused secondaries are not already in kernel mode (i.e. handling a stray
IPI) and then indicate that they should not try to enter the kernel
if they do get woken for any reason.  Instead they will go into KVM code,
find that there is no vcpu to run, acknowledge and clear the IPI and go
back to nap mode.

a8e4ef8414 KVM: PPC: booke: rework rescheduling checks
Instead of checking whether we should reschedule only when we exited
due to an interrupt, let's always check before entering the guest back
again. This gets the target more in line with the other archs.

Also while at it, generalize the whole thing so that eventually we could
have a single kvmppc_prepare_to_enter function for all ppc targets that
does signal and reschedule checking for us.

called msgsnd. This patch emulates this instruction, its clearing counterpart
and the infrastructure required to actually trigger that interrupt inside

d30f6e4800 KVM: PPC: booke: category E.HV (GS-mode) support
Chips such as e500mc that implement category E.HV in Power ISA 2.06
provide hardware virtualization features, including a new MSR mode for
guest state.  The guest OS can perform many operations without trapping
into the hypervisor, including transitions to and from guest userspace.

Since we can use SRR1[GS] to reliably tell whether an exception came from
guest state, instead of messing around with IVPR, we use DO_KVM similarly
to book3s.

Current issues include:
 - Machine checks from guest state are not routed to the host handler.
 - The guest can cause a host oops by executing an emulated instruction
   in a page that lacks read permission.  Existing e500/4xx support has
   the same problem.

2246f8b563 KVM: PPC: Rework wqp conditional code
On PowerPC, we sometimes use a waitqueue per core, not per thread,
so we can't always use the vcpu internal waitqueue.

This code has been generalized by Christoffer Dall recently, but
unfortunately broke compilation for PowerPC. At the time the helper
function is defined, struct kvm_vcpu is not declared yet, so we can't
dereference it.

This patch moves all logic into the generic inline function, at which
time we have all information necessary.

The kvm_vcpu_kick function performs roughly the same funcitonality on
most all architectures, so we shouldn't have separate copies.

PowerPC keeps a pointer to interchanging waitqueues on the vcpu_arch
structure and to accomodate this special need a
__KVM_HAVE_ARCH_VCPU_GET_WQ define and accompanying function
kvm_arch_vcpu_wq have been defined. For all other architectures this
is a generic inline that just returns &vcpu->wq;

3b588c7efc powerpc/epapr: add "memory" as a clobber to all hypercalls
The "memory" clobber tells the compiler to ensure that all writes to memory
are committed before the hypercall is made.

"memory" is only necessary for hcalls where the Hypervisor will read or
write guest memory. However, we add it to all hcalls because the impact is
minimal, and we want to ensure that it's present for the hcalls that need
it.

9cb6abcb26 powerpc/8xxx: remove 85xx/86xx restrictions from fsl_guts.h
Remove the check for CONFIG_PPC_85xx and CONFIG_PPC_86xx from fsl_guts.h.
The check was originally intended to allow the same header file to
be used on 85xx and 86xx systems, even though the Global Utilities
register could be different.  It turns out that they're not actually
different, and so the check is not necessary.  In addition, neither
macro is defined for 64-bit e5500 kernels, so that causes a build
break.

1ce447b90f powerpc/perf: Fix instruction address sampling on 970 and Power4
970 and Power4 don't support "continuous sampling" which means that
when we aren't in marked instruction sampling mode (marked events),
SIAR isn't updated with the last instruction sampled before the
perf interrupt. On those processors, we must thus use the exception
SRR0 value as the sampled instruction pointer.

Those processors also don't support the SIPR and SIHV bits in MMCRA
which means we need some kind of heuristic to decide if SIAR values
represent kernel or user addresses.

     new backend for it on top of some new different type of firwmare.

     The work isn't complete yet, but a good chunk of the cleanups is
     there.  Note that this adds a field to struct device_node which is
     not very nice and which Grant objects to.  I will have a patch soon
     that moves that to a powerpc private data structure (hopefully
     before rc1) and we'll improve things further later on (hopefully
     getting rid of the need for that pointer completely).  Thanks Gavin
     Shan.

   - I dug into our exception & interrupt handling code to improve the
     way we do lazy interrupt handling (and make it work properly with
     "edge" triggered interrupt sources), and while at it found & fixed
     a wagon of issues in those areas, including adding support for page
     fault retry & fatal signals on page faults.

   - Your usual random batch of small fixes & updates, including a bunch
     of new embedded boards, both Freescale and APM based ones, etc..."

38973ba790 powerpc/PCI: compute I/O space bus-to-resource offset consistently
Make sure we compute CPU addresses (resource start/end) the same way both
when we set up the I/O aperture (hose->io_resource) and when we use
pcibios_bus_to_resource() to convert BAR values into resources.

This fixes a build failure ("cast from pointer to integer of different
size" in configs where resource_size_t is 64 bits but pointers are 32 bits)
I introduced in 6c5705fec6.

8626816e90 powerpc: add support for MPIC message register API
Some MPIC implementations contain one or more blocks of message registers
that are used to send messages between cores via IPIs.  A simple API has
been added to access (get/put, read, write, etc ...) these message registers.
The available message registers are initially discovered via nodes in the
device tree.  A separate commit contains a binding for the message register
nodes.

7230c56441 powerpc: Rework lazy-interrupt handling
The current implementation of lazy interrupts handling has some
issues that this tries to address.

We don't do the various workarounds we need to do when re-enabling
interrupts in some cases such as when returning from an interrupt
and thus we may still lose or get delayed decrementer or doorbell
interrupts.

The current scheme also makes it much harder to handle the external
"edge" interrupts provided by some BookE processors when using the
EPR facility (External Proxy) and the Freescale Hypervisor.

Additionally, we tend to keep interrupts hard disabled in a number
of cases, such as decrementer interrupts, external interrupts, or
when a masked decrementer interrupt is pending. This is sub-optimal.

This is an attempt at fixing it all in one go by reworking the way
we do the lazy interrupt disabling from the ground up.

The base idea is to replace the "hard_enabled" field with a
"irq_happened" field in which we store a bit mask of what interrupt
occurred while soft-disabled.

When re-enabling, either via arch_local_irq_restore() or when returning
from an interrupt, we can now decide what to do by testing bits in that
field.

We then implement replaying of the missed interrupts either by
re-using the existing exception frame (in exception exit case) or via
the creation of a new one from an assembly trampoline (in the
arch_local_irq_enable case).

This removes the need to play with the decrementer to try to create
fake interrupts, among others.

In addition, this adds a few refinements:

 - We no longer  hard disable decrementer interrupts that occur
while soft-disabled. We now simply bump the decrementer back to max
(on BookS) or leave it stopped (on BookE) and continue with hard interrupts
enabled, which means that we'll potentially get better sample quality from
performance monitor interrupts.

 - Timer, decrementer and doorbell interrupts now hard-enable
shortly after removing the source of the interrupt, which means
they no longer run entirely hard disabled. Again, this will improve
perf sample quality.

 - On Book3E 64-bit, we now make the performance monitor interrupt
act as an NMI like Book3S (the necessary C code for that to work
appear to already be present in the FSL perf code, notably calling
nmi_enter instead of irq_enter). (This also fixes a bug where BookE
perfmon interrupts could clobber r14 ... oops)

 - We could make "masked" decrementer interrupts act as NMIs when doing
timer-based perf sampling to improve the sample quality.

3780444c4f powerpc/eeh: pseries platform config space access in EEH
With the original EEH implementation, the access to config space of
the corresponding PCI device is done by RTAS sensitive function. That
depends on pci_dn heavily. That would limit EEH extension to other
platforms like powernv because other platforms might have different
ways to access PCI config space.

The patch splits those functions used to access PCI config space
and implement them in platform related EEH component. It would be
helpful to support EEH on multiple platforms simutaneously in future.

We have to put EEH related information to pci_dn. Actually, we could
split struct pci_dn so that the EEH sensitive information to form an
individual struct, then EEH looks more independent.

The patch replaces pci_dn with eeh_dev for EEH aux components like
event and driver. Also, the eeh_event struct has been adjusted for
a little bit since eeh_dev has linked the associated FDT (Flat Device
Tree) node and PCI device. It's not necessary for eeh_event struct to
trace FDT node and PCI device. We can just simply to trace eeh_dev in
eeh_event.

The patch also renames function pcid_name() to eeh_pcid_name(), which
should be missed in the previous patch where the EEH aux components
have been cleaned up.

eb740b5f3e powerpc/eeh: Introduce EEH device
Original EEH implementation depends on struct pci_dn heavily. However,
EEH shouldn't depend on that actually because EEH needn't share much
information with other PCI components. That's to say, EEH should have
worked independently.

The patch introduces struct eeh_dev so that EEH core components needn't
be working based on struct pci_dn in future. Also, struct pci_dn, struct
eeh_dev instances are created in dynamic fasion and the binding with EEH
device, OF node, PCI device is implemented as well.

The EEH devices are created after PHBs are detected and initialized, but
PCI emunation hasn't started yet. Apart from that, PHB might be created
dynamically through DLPAR component and the EEH devices should be creatd
as well. Another case might be OF node is created dynamically by DR
(Dynamic Reconfiguration), which has been defined by PAPR. For those OF
nodes created by DR, EEH devices should be also created accordingly. The
binding between EEH device and OF node is done while the EEH device is
initially created.

The binding between EEH device and PCI device should be done after PCI
emunation is done. Besides, PCI hotplug also needs the binding so that
the EEH devices could be traced from the newly coming PCI buses or PCI
devices.

particular PE should be enabled when doing recovery on EEH errors
so that the PE could function properly again.

The patch implements it and abstract that through struct

aa1e6374ae powerpc/eeh: Platform dependent EEH operations
EEH has been implemented on RTAS-compliant pSeries platform.
That's to say, the EEH operations will be implemented through RTAS
calls eventually. The situation limited feasible extension on EEH.
In order to support EEH on multiple platforms like pseries and powernv
simutaneously. We have to split the platform dependent EEH options
up out of current implementation.

The patch addresses supporting EEH on multiple platforms. The pseries
platform dependent EEH operations will be abstracted by struct eeh_ops.
EEH core components will be built based on the registered EEH operations.
With the mechanism, what the individual platform needs to do is implement
platform dependent EEH operations.

For now, the pseries platform is covered under the mechanism. That means
we have to think about other platforms to support EEH, like powernv.
Besides, we only have framework for the mechanism and we have to implement
it for pseries platform later.

we may call C code from the macro EXCEPTION_PROLOG_COMMON in the
exception entry path, which clobbers CR0.

However, the FPU and vector traps rely on CR0 indicating whether we
are coming from userspace or kernel to decide what to do.

So we need to restore that value after the C call

a546498f3b powerpc: Call do_page_fault() with interrupts off
We currently turn interrupts back to their previous state before
calling do_page_fault(). This can be annoying when debugging as
a bad fault will potentially have lost some processor state before
getting into the debugger.

We also end up calling some generic code with interrupts enabled
such as notify_page_fault() with interrupts enabled, which could
be unexpected.

This changes our code to behave more like other architectures,
and make the assembly entry code call into do_page_faults() with
interrupts disabled. They are conditionally re-enabled from
within do_page_fault() in the same spot x86 does it.

While there, add the might_sleep() test in the case of a successful
trylock of the mmap semaphore, again like x86.

Also fix a bug in the existing assembly where r12 (_MSR) could get
clobbered by C calls (the DTL accounting in the exception common
macro and DISABLE_INTS) in some cases.

1b70117924 powerpc: Improve behaviour of irq tracing on 64-bit exception entry
Some exceptions would unconditionally disable interrupts on entry,
which is fine, but calling lockdep every time not only adds more
overhead than strictly needed, but also means we get quite a few
"redudant" disable logged, which makes it hard to spot the really
bad ones.

So instead, split the macro used by the exception code into a
normal one and a separate one used when CONFIG_TRACE_IRQFLAGS is
enabled, and make the later skip th tracing if interrupts were
already disabled.

fe1952fc0a powerpc: Rework runlatch code
This moves the inlines into system.h and changes the runlatch
code to use the thread local flags (non-atomic) rather than
the TIF flags (atomic) to keep track of the latch state.

The code to turn it back on in an asynchronous interrupt is
now simplified and partially inlined.

7450f6f03e powerpc: Use the same interrupt prolog for perfmon as other interrupts
The perfmon interrupt is the sole user of a special variant of the
interrupt prolog which differs from the one used by external and timer
interrupts in that it saves the non-volatile GPRs and doesn't turn the
runlatch on.

The former is unnecessary and the later is arguably incorrect, so
let's clean that up by using the same prolog. While at it we rename
that prolog to use the _ASYNC prefix.

moment we use atomic*_add_unless which requires us to put 0 and

d2a1b483a4 KVM: PPC: Add HPT preallocator
We're currently allocating 16MB of linear memory on demand when creating
a guest. That does work some times, but finding 16MB of linear memory
available in the system at runtime is definitely not a given.

So let's add another command line option similar to the RMA preallocator,
that we can use to keep a pool of page tables around. Now, when a guest
gets created it has a pretty low chance of receiving an OOM.

31f3438eca KVM: PPC: Move kvm_vcpu_ioctl_[gs]et_one_reg down to platform-specific code
This moves the get/set_one_reg implementation down from powerpc.c into
booke.c, book3s_pr.c and book3s_hv.c.  This avoids #ifdefs in C code,
but more importantly, it fixes a bug on Book3s HV where we were
accessing beyond the end of the kvm_vcpu struct (via the to_book3s()
macro) and corrupting memory, causing random crashes and file corruption.

On Book3s HV we only accept setting the HIOR to zero, since the guest
runs in supervisor mode and its vectors are never offset from zero.

Until now, we always set HIOR based on the PVR, but this is just wrong.
Instead, we should be setting HIOR explicitly, so user space can decide
what the initial HIOR value is - just like on real hardware.

We keep the old PVR based way around for backwards compatibility, but
once user space uses the SET_ONE_REG based method, we drop the PVR logic.

82ed36164c KVM: PPC: Book3s HV: Implement get_dirty_log using hardware changed bit
This changes the implementation of kvm_vm_ioctl_get_dirty_log() for
Book3s HV guests to use the hardware C (changed) bits in the guest
hashed page table.  Since this makes the implementation quite different
from the Book3s PR case, this moves the existing implementation from
book3s.c to book3s_pr.c and creates a new implementation in book3s_hv.c.
That implementation calls kvmppc_hv_get_dirty_log() to do the actual
work by calling kvm_test_clear_dirty on each page.  It iterates over
the HPTEs, clearing the C bit if set, and returns 1 if any C bit was
set (including the saved C bit in the rmap entry).

see if any of them have R set.

bad3b5075e KVM: PPC: Book3s HV: Maintain separate guest and host views of R and C bits
This allows both the guest and the host to use the referenced (R) and
changed (C) bits in the guest hashed page table.  The guest has a view
of R and C that is maintained in the guest_rpte field of the revmap
entry for the HPTE, and the host has a view that is maintained in the
rmap entry for the associated gfn.

Both view are updated from the guest HPT.  If a bit (R or C) is zero
in either view, it will be initially set to zero in the HPTE (or HPTEs),
until set to 1 by hardware.  When an HPTE is removed for any reason,
the R and C bits from the HPTE are ORed into both views.  We have to
be careful to read the R and C bits from the HPTE after invalidating
it, but before unlocking it, in case of any late updates by the hardware.

With this, if a guest does an H_ENTER with a read/write HPTE on a page
which is currently read-only, we make the actual HPTE inserted be a
read-only version of the HPTE.  We now intercept protection faults as
well as HPTE not found faults, and for a protection fault we work out
whether it should be reflected to the guest (e.g. because the guest HPTE
didn't allow write access to usermode) or handled by switching to
kernel context and calling kvmppc_book3s_hv_page_fault, which will then
request write access to the page and update the actual HPTE.

342d3db763 KVM: PPC: Implement MMU notifiers for Book3S HV guests
This adds the infrastructure to enable us to page out pages underneath
a Book3S HV guest, on processors that support virtualized partition
memory, that is, POWER7.  Instead of pinning all the guest's pages,
we now look in the host userspace Linux page tables to find the
mapping for a given guest page.  Then, if the userspace Linux PTE
gets invalidated, kvm_unmap_hva() gets called for that address, and
we replace all the guest HPTEs that refer to that page with absent
HPTEs, i.e. ones with the valid bit clear and the HPTE_V_ABSENT bit
set, which will cause an HDSI when the guest tries to access them.
Finally, the page fault handler is extended to reinstantiate the
guest HPTE when the guest tries to access a page which has been paged
out.

Since we can't intercept the guest DSI and ISI interrupts on PPC970,
we still have to pin all the guest pages on PPC970.  We have a new flag,
kvm->arch.using_mmu_notifiers, that indicates whether we can page
guest pages out.  If it is not set, the MMU notifier callbacks do
nothing and everything operates as before.

697d3899dc KVM: PPC: Implement MMIO emulation support for Book3S HV guests
This provides the low-level support for MMIO emulation in Book3S HV
guests.  When the guest tries to map a page which is not covered by
any memslot, that page is taken to be an MMIO emulation page.  Instead
of inserting a valid HPTE, we insert an HPTE that has the valid bit
clear but another hypervisor software-use bit set, which we call
HPTE_V_ABSENT, to indicate that this is an absent page.  An
absent page is treated much like a valid page as far as guest hcalls
(H_ENTER, H_REMOVE, H_READ etc.) are concerned, except of course that
an absent HPTE doesn't need to be invalidated with tlbie since it
was never valid as far as the hardware is concerned.

When the guest accesses a page for which there is an absent HPTE, it
will take a hypervisor data storage interrupt (HDSI) since we now set
the VPM1 bit in the LPCR.  Our HDSI handler for HPTE-not-present faults
looks up the hash table and if it finds an absent HPTE mapping the
requested virtual address, will switch to kernel mode and handle the
fault in kvmppc_book3s_hv_page_fault(), which at present just calls
kvmppc_hv_emulate_mmio() to set up the MMIO emulation.

This is based on an earlier patch by Benjamin Herrenschmidt, but since
heavily reworked.

06ce2c63d9 KVM: PPC: Maintain a doubly-linked list of guest HPTEs for each gfn
This expands the reverse mapping array to contain two links for each
HPTE which are used to link together HPTEs that correspond to the
same guest logical page.  Each circular list of HPTEs is pointed to
by the rmap array entry for the guest logical page, pointed to by
the relevant memslot.  Links are 32-bit HPT entry indexes rather than
full 64-bit pointers, to save space.  We use 3 of the remaining 32
bits in the rmap array entries as a lock bit, a referenced bit and
a present bit (the present bit is needed since HPTE index 0 is valid).
The bit lock for the rmap chain nests inside the HPTE lock bit.

9d0ef5ea04 KVM: PPC: Allow I/O mappings in memory slots
This provides for the case where userspace maps an I/O device into the
address range of a memory slot using a VM_PFNMAP mapping.  In that
case, we work out the pfn from vma->vm_pgoff, and record the cache
enable bits from vma->vm_page_prot in two low-order bits in the
slot_phys array entries.  Then, in kvmppc_h_enter() we check that the
cache bits in the HPTE that the guest wants to insert match the cache
bits in the slot_phys array entry.  However, we do allow the guest to
create what it thinks is a non-cacheable or write-through mapping to
memory that is actually cacheable, so that we can use normal system
memory as part of an emulated device later on.  In that case the actual
HPTE we insert is a cacheable HPTE.

This relaxes the requirement that the guest memory be provided as
16MB huge pages, allowing it to be provided as normal memory, i.e.
in pages of PAGE_SIZE bytes (4k or 64k).  To allow this, we index
the kvm->arch.slot_phys[] arrays with a small page index, even if
huge pages are being used, and use the low-order 5 bits of each
entry to store the order of the enclosing page with respect to
normal pages, i.e. log_2(enclosing_page_size / PAGE_SIZE).

c77162dee7 KVM: PPC: Only get pages when actually needed, not in prepare_memory_region()
This removes the code from kvmppc_core_prepare_memory_region() that
looked up the VMA for the region being added and called hva_to_page
to get the pfns for the memory.  We have no guarantee that there will
be anything mapped there at the time of the KVM_SET_USER_MEMORY_REGION
ioctl call; userspace can do that ioctl and then map memory into the
region later.

Instead we defer looking up the pfn for each memory page until it is
needed, which generally means when the guest does an H_ENTER hcall on
the page.  Since we can't call get_user_pages in real mode, if we don't
already have the pfn for the page, kvmppc_h_enter() will return
H_TOO_HARD and we then call kvmppc_virtmode_h_enter() once we get back
to kernel context.  That calls kvmppc_get_guest_page() to get the pfn
for the page, and then calls back to kvmppc_h_enter() to redo the HPTE
insertion.

When the first vcpu starts executing, we need to have the RMO or VRMA
region mapped so that the guest's real mode accesses will work.  Thus
we now have a check in kvmppc_vcpu_run() to see if the RMO/VRMA is set
up and if not, call kvmppc_hv_setup_rma().  It checks if the memslot
starting at guest physical 0 now has RMO memory mapped there; if so it
sets it up for the guest, otherwise on POWER7 it sets up the VRMA.
The function that does that, kvmppc_map_vrma, is now a bit simpler,
as it calls kvmppc_virtmode_h_enter instead of creating the HPTE itself.

Since we are now potentially updating entries in the slot_phys[]
arrays from multiple vcpu threads, we now have a spinlock protecting
those updates to ensure that we don't lose track of any references
to pages.

075295dd32 KVM: PPC: Make the H_ENTER hcall more reliable
At present, our implementation of H_ENTER only makes one try at locking
each slot that it looks at, and doesn't even retry the ldarx/stdcx.
atomic update sequence that it uses to attempt to lock the slot.  Thus
it can return the H_PTEG_FULL error unnecessarily, particularly when
the H_EXACT flag is set, meaning that the caller wants a specific PTEG
slot.

This improves the situation by making a second pass when no free HPTE
slot is found, where we spin until we succeed in locking each slot in
turn and then check whether it is full while we hold the lock.  If the
second pass fails, then we return H_PTEG_FULL.

This also moves lock_hpte to a header file (since later commits in this
series will need to use it from other source files) and renames it to
try_lock_hpte, which is a somewhat less misleading name.

This adds two new functions, kvmppc_pin_guest_page() and
kvmppc_unpin_guest_page(), and uses them to pin the guest pages where
the guest has registered areas of memory for the hypervisor to update,
(i.e. the per-cpu virtual processor areas, SLB shadow buffers and
dispatch trace logs) and then unpin them when they are no longer
required.

Although it is not strictly necessary to pin the pages at this point,
since all guest pages are already pinned, later commits in this series
will mean that guest pages aren't all pinned.

b2b2f16508 KVM: PPC: Keep page physical addresses in per-slot arrays
This allocates an array for each memory slot that is added to store
the physical addresses of the pages in the slot.  This array is
vmalloc'd and accessed in kvmppc_h_enter using real_vmalloc_addr().
This allows us to remove the ram_pginfo field from the kvm_arch
struct, and removes the 64GB guest RAM limit that we had.

We use the low-order bits of the array entries to store a flag
indicating that we have done get_page on the corresponding page,
and therefore need to call put_page when we are finished with the
page.  Currently this is set for all pages except those in our
special RMO regions.

8936dda4c2 KVM: PPC: Keep a record of HV guest view of hashed page table entries
This adds an array that parallels the guest hashed page table (HPT),
that is, it has one entry per HPTE, used to store the guest's view
of the second doubleword of the corresponding HPTE.  The first
doubleword in the HPTE is the same as the guest's idea of it, so we
don't need to store a copy, but the second doubleword in the HPTE has
the real page number rather than the guest's logical page number.
This allows us to remove the back_translate() and reverse_xlate()
functions.

This "reverse mapping" array is vmalloc'd, meaning that to access it
in real mode we have to walk the kernel's page tables explicitly.
That is done by the new real_vmalloc_addr() function.  (In fact this
returns an address in the linear mapping, so the result is usable
both in real mode and in virtual mode.)

There are also some minor cleanups here: moving the definitions of
HPT_ORDER etc. to a header file and defining HPT_NPTE for HPT_NPTEG << 3.

reduces conflicts, especially on e500v2 where a single guest

When running the 64-bit Book3s PR code without CONFIG_PREEMPT_NONE, we were
doing a few things wrong, most notably access to PACA fields without making
sure that the pointers stay stable accross the access (preempt_disable()).

This patch moves to_svcpu towards a get/put model which allows us to disable
preemption while accessing the shadow vcpu fields in the PACA. That way we
can run preemptible and everyone's happy!

Decrementers are now properly driven by TCR/TSR, and the guest
has full read/write access to these registers.

The decrementer keeps ticking (and setting the TSR bit) regardless of
whether the interrupts are enabled with TCR.

The decrementer stops at zero, rather than going negative.

Decrementers (and FITs, once implemented) are delivered as
level-triggered interrupts -- dequeued when the TSR bit is cleared, not
on delivery.

b59049720d KVM: PPC: Paravirtualize SPRG4-7, ESR, PIR, MASn
This allows additional registers to be accessed by the guest
in PR-mode KVM without trapping.

SPRG4-7 are readable from userspace.  On booke, KVM will sync
these registers when it enters the guest, so that accesses from
guest userspace will work.  The guest kernel, OTOH, must consistently
use either the real registers or the shared area between exits.  This
also applies to the already-paravirted SPRG3.

On non-booke, it's not clear to what extent SPRG4-7 are supported
(they're not architected for book3s, but exist on at least some classic
chips).  They are copied in the get/set regs ioctls, but I do not see any
non-booke emulation.  I also do not see any syncing with real registers
(in PR-mode) including the user-readable SPRG3.  This patch should not
make that situation any worse.
