Ingo Molnar 0bfc24559d blktrace: port to tracepoints, update
Port to the new tracepoints API: split DEFINE_TRACE() and DECLARE_TRACE()
sites. Spread them out to the usage sites, as suggested by
Mathieu Desnoyers.
