Toggle the AI1 mux when changing the CX18_AUDIO_ENABLE register. It's hard to
reliably tell when we have written to this register successfully unless we
change some bits we know we can read back. The AI mux bits always read back
what we wrote to them, so force them to toggle whenever we have to write to
the register, so we can tell we wrote to the register successfully.
This change was prompted by users experiencing broadcast audio decoding
problems after the cx18 module loads for the first time.

Add code to verify the cx18-av-core digitizer firmware loads correctly. The
verification function reads back and compares the firmware bytes loaded
into the A/V core. The result of the verification is only used to log a
message in the system log.
This change was prompted by users with multiple card setups that have problems
with broadcast audio decoding the first time the cx18 module is loaded.

Give messages originating from internal subdevs a header using the subdev's
name. Fixed an uninitialized variable problem with reading the EEPROM, noticed
from log output. Got rid of the unused cx18_av_exit() function.

The CX18_AUDIO_ENABLE register usually never reads back what was just written
under normal circumstances. Perform better checking that a write went to the
register as expected with a specification of what bits to verify.

Renamed structure member name to be more specific to type in anticipation
of updating to the v4l2_device/v4l2_subdev framework. Too many objects named
"dev" and /v4l2_\{0,1\}dev/ would be to confusing.

A redundant PLL divisior update for the I2S master clock after AV core
firmware load was missed in earlier PLL parameter changes. This one really
doesn't matter because it's redundant and gets overwritten, but the driver
should be self consistent in the values used.

cx18: Streamline cx18-io[ch] wrappers and enforce MMIO retry strategy so that
write retries always occur and read retries never occur (as they never help).
Remove MMIO statistics logging to speed up MMIO accesses. Deprecate & ignore
retry_mmio and mmio_ndelay module parameters, to essentially force
retry_mmio=1 and mmio_ndelay=0.

cx18: Reduce number of mmio read retries to improve performance. Experiments
have shown 2 things: read retries never improve the result of a suspect mmio
read from the CX23418 (the result stays all 0xff's), and that most of the
suspected read failures are actually proper reads of values that should be
all 0xff's. This change reduces the number of read retries and keeps the
count separate from write retries.

cx18: Fix write retries for registers that always change - part 3.
Fix the io for the rest of the registers that will often not read back the
value just written. Modified register readback checks to make sure the
intended effect was achieved without constantly rewriting the registers.
The one outstanding register remaining is 0xc72014 CX18_AUDIO_ENABLE, whose
behavior on writes I have yet to determine.

cx18: Add default behavior of checking and retrying PCI MMIO accesses.
The concept of checking and retrying PCI MMIO accesses for better reliability
in older motherboards was suggested by Steve Toth <stoth@linuxtv.org>. This
change implements MMIO retries and the retry_mmio module parameter that is
enabled by default. Limited experiments have shown this is more reliable than
the mmio_ndelay parameter. mmio_ndelay has insignificant effect with retries
enabled.
