When the transmit queue gets full we enable interrupts for TX completions
There was a race that we handled the TX queue both from the interrupt context
and from the transmit function. Using "spin_trylock_irq()" ensures this
doesn't happen.

The low level driver always assumes this handler exists.
The lack of it could cause kernel panic

Was trying to unmap work queue entries that had inline packets,
so naturally weren't mapped.

There is no need to synchronize the polling with the transmit
function. The only place to synchronize is when we process
the cq from the transmit function. Also removed spin_lock_irq,
and using spin_trylock, if somebody else is already processing the cq,
no need to wait for it to finish.

The Mellanox ConnectX can operate as an InfiniBand adapter, as an
Ethernet NIC, or as a Fibre Channel (FC) HBA. The kernel has a
low-level driver, mlx4_core, which handles multiplexing access to the
device, and there is also already an InfiniBad driver, mlx4_ib.
This patch adds a new driver, mlx4_en, which implements a standard
Ethernet NIC driver.
