CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

There're couples of functions defined to print debugging messages
during initializing P7IOC. However, we got bogus output from those
functions like pe_info(). The problem here is that the message
level (the first parameter to printk()) isn't printable and that
caused the bogus output.
The patch fixes the issue by merging __pe_printk() to the macro
define_pe_printk_level() so that we can pass the message level
directly to printk().

Currently, the PCI probe flags "PCI_PROBE_ONLY | PCI_REASSIGN_ALL_RSRC"
used on powernv platform. That means the platform has to do the PCI
resource assignment by itself.
The patch changes the PCI probe flag to "PCI_REASSIGN_ALL_RSRC" so
that the PCI core will do the resource assignment. Also, the I/O
and MMIO minimal alignment for P2P bridges have been configured
while doing fixup for the PHBs.

There're 2 arrays introduced to trace which PE has occupied the
corresponding resource (I/O or MMIO) segment. However, we didn't
allocate enough memory for them and that possiblly leads to PE
descriptor corruption.
The patch fixes that by allocating enough memory for those 2 arrays.

While the device driver or PCI core tries to enable PCI device, the
platform dependent callback "ppc_md.pcibios_enable_device_hook" will
be called to check if there has one associated PE for the PCI device.
If we don't have the associated PE for the PCI device, it's not allowed
to enable the PCI device. Unfortunately, there might have some cases
we have to enable the PCI device (e.g. P2P bridge), but the PEs have
not been created yet.
The patch handles the unfortunate cases. Each PHB (struct pnv_phb)
has one field "initialized" to trace if the PEs have been created
and configured or not. When the PEs are not available, we won't check
the associated PE for the PCI device to be enabled.

There're 2 types of PCI bus sensitive PEs: (A) The PE includes
single PCI bus. (B) The PE includes the PCI bus and all the subordinate
PCI buses, and the patch tries to assign I/O and MMIO resources
based on created PEs. Fortunately, we figured out unified scheme
to do resource assignment for all types of PCI bus based PEs according
to Ben's idea:
- Resource assignment based on PE from top to bottom.
- The soureces, either I/O or MMIO, of the PE are figured out
from the assigned PCI bus.
- The occupied resource by parent PE could possibilly be overrided
by children PEs.

The resource (I/O and MMIO) will be assigned on basis of PE from
top to bottom so that we can implement the trick here: the resource
that has been assigned to parent PE could be taken by child PE if
necessary.
The current implementation already has PE list per PHB basis, but
the list doesn't meet our requirment: tracing PE based on their
cration time from top to bottom. So the patch does rename for the
DMA based PE list and introduces the list to trace the PEs sequentially
based on their creation time.

Basically, there're 2 types of PCI bus sensitive PEs: (A) The PE
includes single PCI bus. (B) The PE includes the PCI bus and all
the subordinate PCI buses. At present, we'd like to put PCI bus
originated by PCI-e link to form PE that contains single PCI bus,
and the PCIe-to-PCI bridge will form the 2nd type of PE. We don't
figure out to detect PLX bridge yet. Once we can detect PLX bridge
some day, we have to put PCI buses originated from the downstream
port of PLX bridge to the 2nd type of PE.
The patch changes the original implementation for a little bit
to support 2 types of PCI bus sensitive PEs described as above.
Also, the function used to retrieve the corresponding PE according
to the given PCI device has been changed based on that because each
PCI device should trace the directly associated PE.

The patch implements ppc_md.pcibios_window_alignment for powernv
platform so that the resource reassignment in PCI core will be
done according to the I/O and memory alignment returned from
powernv platform. The alignments returned from powernv platform
is closely depending on the scheme for PE segmenting. Besides,
the patch isn't useful for now, but the subsequent patches will
be working based on it.

The CPU hotplug code for the powernv platform currently only puts
offline CPUs into nap mode if the powersave_nap variable is set.
However, HV-style KVM on this platform requires secondary CPU threads
to be offline and in nap mode. Since we know nap mode works just
fine on all POWER7 machines, and the only machines that support the
powernv platform are POWER7 machines, this changes the code to
always put offline CPUs into nap mode, regardless of powersave_nap.
Powersave_nap still controls whether or not CPUs go into nap mode
when idle, as before.

Anything that uses a constructed instruction (ie. from ppc-opcode.h),
need to use the new R0 macro, as %r0 is not going to work.
Also convert usages of macros where we are just determining an offset
(usually for a load/store), like:
std r14,STK_REG(r14)(r1)
Can't use STK_REG(r14) as %r14 doesn't work in the STK_REG macro since
it's just calculating an offset.

We already use pci_flags, so this just sets pci_flags directly and removes
the intermediate step of figuring out pci_probe_only, then using it to set
pci_flags.
The PCI core provides a pci_flags definition (currently __weak), so drop
the powerpc definitions in favor of that.

pci_probe_only is set on ppc64 to prevent resource re-allocation
by the core. It's meant to be used in very specific circumstances
such as when operating under a hypervisor that may prevent such
re-allocation.
Instead of default to 1, we make it default to 0 and explicitly
set it in the few cases where we need it.
This fixes FSL PCI which wants it clear among others.

Recent changes to the handling of PCI resources for host bridges
are breaking the PowerNV code for assigning resources on IODA.
The root of the problem is that the pci_bus attached to a host
bridge no longer has its "legacy" resource pointers populated
but only uses the newer list instead.

At present, on the powernv platform, if you off-line a CPU that was
online, and then try to on-line it again, the kernel generates a
warning message "OPAL Error -1 starting CPU n". Furthermore, if the
CPU is a secondary thread that was used by KVM while it was off-line,
the CPU fails to come online.
The first problem is fixed by only calling OPAL to start the CPU the
first time it is on-lined, as indicated by the cpu_start field of its
PACA being zero. The second problem is fixed by restoring the
cpu_start field to 1 instead of 0 when using the CPU within KVM.

This adds support for p7IOC (and possibly other IODA v1 IO Hubs)
using OPAL v2 interfaces.
We completely take over resource assignment and assign them using an
algorithm that hands out device BARs in a way that makes them fit in
individual segments of the M32 window of the bridge, which enables us
to assign individual PEs to devices and functions.
The current implementation gives out a PE per functions on PCIe, and a
PE for the entire bridge for PCIe to PCI-X bridges.
This can be adjusted / fine tuned later.
We also setup DMA resources (32-bit only for now) and MSIs (both 32-bit
and 64-bit MSI are supported).
The DMA allocation tries to divide the available 256M segments of the
32-bit DMA address space "fairly" among PEs. This is done using a
"weight" heuristic which assigns less value to things like OHCI USB
controllers than, for example SCSI RAID controllers. This algorithm
will probably want some fine tuning for specific devices or device
types.

The firmware doesn't wait after lifting the PCI reset. However it does
timestamp it in the device tree. We use that to ensure we wait long
enough (3s is our current arbitrary setting) from that timestamp to
actually probing the bus.

This implements support for MSIs on p5ioc2 PHBs. We only support
MSIs on the PCIe PHBs, not the PCI-X ones as the later hasn't been
properly verified in HW.

This adds support for PCI-X and PCIe on the p5ioc2 IO hub using
OPAL. This includes allocating & setting up TCE tables and config
space access routines.
This also supports fallbacks via RTAS when OPAL is absent, using
legacy TCE format pre-allocated via the device-tree (BML style)

OPAL can handle various interrupt for us such as Machine Checks (it
performs all sorts of recovery tasks and passes back control to us with
informations about the error), Hardware Management Interrupts and Softpatch
interrupts.
This wires up the mechanisms and prints out specific informations returned
by HAL when a machine check occurs.

makes the console smoother and will allow us to implement
interrupt based completion and console.

Implements OPAL RTC and NVRAM support and wire all that up to
the powernv platform.
We use RTAS for RTC as a fallback if available. Using RTAS for nvram
is not supported yet, pending some rework/cleanup and generalization
of the pSeries & CHRP code. We also use RTAS fallbacks for power off
and reboot

This calls the respective HAL functions, and spin on hal_poll_event()
to ensure the HAL has a chance to communicate with the FSP to trigger
the reboot or shutdown operation

This adds a udbg and an hvc console backend for supporting a console
using the OPAL console interfaces.
On OPAL v1 we have hvc0 mapped to whatever console the system was
configured for (network or hvsi serial port) via the service
processor.
On OPAL v2 we have hvcN mapped to the Nth console provided by OPAL
which generally corresponds to:
hvc0 : network console (raw protocol)
hvc1 : serial port S1 (hvsi)
hvc2 : serial port S2 (hvsi)
Note: At this point, early debug console only works with OPAL v1
and shouldn't be enabled in a normal kernel.

Add definition of OPAL interfaces along with the wrappers to call
into OPAL runtime and the early device-tree parsing hook to locate
the OPAL runtime firmware.

On machines supporting the OPAL firmware version 1, the system
is initially booted under pHyp. We then use a special hypercall
to verify if OPAL is available and if it is, we then trigger
a "takeover" which disables pHyp and loads the OPAL runtime
firmware, giving control to the kernel in hypervisor mode.
This patch add the necessary code to detect that the OPAL takeover
capability is present when running under PowerVM (aka pHyp) and
perform said takeover to get hypervisor control of the processor.
To perform the takeover, we must first use RTAS (within Open
Firmware runtime environment) to start all processors & threads,
in order to give control to OPAL on all of them. We then call
the takeover hypercall on everybody, OPAL will re-enter the kernel
main entry point passing it a flat device-tree.

This adds a skeletton for the new Power "Non Virtualized"
platform which will be used by machines supporting running
without an hypervisor, for example in order to run KVM.
These machines will be using a new firmware called OPAL
for which the support will be provided by later patches.
The PowerNV platform is intended to be also usable under
the BML environment used internally for early CPU bringup
which is why the code also supports using RTAS instead of
OPAL in various places.
