5101730cb0 x86, mm: use pfn_range_is_mapped() with gart
We are going to map ram only, so under max_low_pfn_mapped,
between 4g and max_pfn_mapped does not mean mapped at all.

Use pfn_range_is_mapped() directly.

395cf9691d doc: fix broken references
There are numerous broken references to Documentation files (in other
Documentation files, in comments, etc.). These broken references are
caused by typo's in the references, and by renames or removals of the
Documentation files. Some broken references are simply odd.

Fix these broken references, sometimes by dropping the irrelevant text
they were part of.

fffcda1183 x86, gart: Rename pci-gart_64.c to amd_gart_64.c
This file only contains code relevant for the northbridge
gart in AMD processors. This patch renames the file to
represent this fact in the filename.
