Subject: [PATCH] KVM: Trivial: Use standard BITMAP macros, open-code
 userspace-exposed header

Creating one's own BITMAP macro seems suboptimal: if we use manual
arithmetic in the one place exposed to userspace, we can use standard
macros elsewhere.
