[ARM] Remove MODE_(SVC|IRQ|FIQ|USR) and DEFAULT_FIQ
DEFAULT_FIQ was entirely unused. MODE_* are just redefinitions of *_MODE. Use *_MODE instead. Signed-off-by: Russell King <rmk+kernel@arm.linux.org.uk>
