USB: EHCI: tegra: Drop unused defines

Since commit 2d22b42db0 "usb: phy: registering Tegra USB PHY as
platform driver" the driver no longer relies on the hard-coded physical
addresses to determine the association between PHY and EHCI port, so
these defines can be dropped.
