[PATCH] Add NOPFN_REFAULT result from vm_ops->nopfn()

Add a NOPFN_REFAULT return code for vm_ops->nopfn() equivalent to
NOPAGE_REFAULT for vmops->nopage() indicating that the handler requests a
re-execution of the faulting instruction
